While you are in hospital, we can arrange for the right level of support to be put in place for a smooth return home, allowing those final days to be spent in the environment you know and love best.

Support Workers can also provide reassurance and comfort, both to you and your loved one. Alongside supporting other citizens at the end of their lives, many of our support workers have also had personal experience in this area. They offer a valuable source of support and comfort, having that personal understanding of what can be a distressing and difficult time.

Complex needs

For more complex healthcare needs, whether it is more complex medication or some form of clinical intervention such as a peg-feeding tube or tracheotomy, our team of clinical specialists will set-up and manage this support for you.

A fully trained support worker can offer support with many tasks to make you or a loved one feel comfortable and  during what can be a distressing time.

 

This includes:

Personal care – supporting with bathing and showering, toileting, including changing continence pads or using a commode

Mobility support, such as help moving position in bed and help with standing or sitting – and using relevant equipment when required

Medication administering or prompting, supporting with pain management

Changing bedding and housekeeping duties

Preparing meals and snacks to taste and dietary requirements

Complex care such as changing catheters, stomps and PEG feeding

Companionship and emotional support
